About the role:

We are seeking an experienced Allied Health Senior Clinician (Occupational Therapist or Physiotherapist) to provide clinical leadership in the care of older people receiving treatment through our Admitted at Home programs. This exciting position has a dual role both within their discipline and as a transdisciplinary clinician providing comprehensive assessment and management, supporting older adults to receive safe, timely, and high-quality care in their own homes.


The role focuses on facilitating early discharge, preventing avoidable hospital admissions, and connecting patients and families with appropriate community supports to promote independence and positive outcomes.


Key responsibilities:

Lead transdisciplinary physical, cognitive, and psychosocial screening, assessment, and intervention for older people in admitted-at-home programs.
Manage a complex clinical caseload, providing expert clinical input to care planning, discharge, and care transitions.
Apply advanced clinical reasoning and shared decision-making to optimise functional outcomes and support safe care at home.
Contribute to service leadership, operational priorities, and quality improvement initiatives.
Support workforce development through education, mentoring, and modelling of evidence-based, transdisciplinary practice.

About you:

Seven years of postgraduate experience
Experience in providing care in the home environment
Demonstrated leadership skills and extensive experience in the supervision and professional development of physiotherapists, students and support staff
Demonstrated ability to communicate and work effectively in a multidisciplinary team environment
