POSITION SUMMARY

The Lead Personal Care Assistant is responsible for coordinating and overseeing caregiver assignments to ensure the equitable distribution of resident care responsibilities throughout the community. This role involves daily review and adjustment of caregiver assignments based on resident needs, ensuring that tasks are manageable and balanced across the team. As resident acuity levels change, particularly with new move-ins requiring increased care, the position ensures that staffing is adapted accordingly, promoting efficiency and high-quality care delivery. The Lead Personal Care Assistant plays a critical role in maintaining consistent coverage and supporting caregivers in meeting the evolving needs of residents within their designated areas.

Essential Duties And Responsibilities

In addition to all components of the above position summary the following “Essential Duties and Responsibilities” are essential to successful performance in this position
• Ensure medical supplies are always in stock. Let the Wellness Coordinator know when supplies are running low.
• Monitor and ensure adherence to weekly bath schedules, verifying that all scheduled baths are completed. Investigate and document any missed baths, including identifying whether refusals are due to resident choice or staffing concerns.
• Begin each shift by reviewing and communicating the shift’s priorities and responsibilities. Conclude each shift with a brief handoff, outlining any unresolved tasks and resident concerns to ensure continuity of care.
• Ensure that a final round is made by all caregivers before leaving to ensure that trash has been taken out, meal trays are not outside of apartments.
• Review the daily meal list to ensure all residents have received or been accounted for during meals. Verify and update the list of residents receiving trays, and communicate any changes to the culinary department, including residents no longer in isolation, those requiring in-room meals due to illness, or those needing a packed meal while off premises.…
• Meet with the head of housekeeping to ensure that housekeeping and laundry was completed for the shift, and if not facilitate assistance with caregivers in completing these tasks.
• Communicate updates to all caregivers regarding new resident move-ins, potential move-outs, and the status of any ongoing or recently resolved resident-related concerns.
• Immediately report any caregiver call-offs or no-shows to the Resident Services Director prior to the end of the previous shift to allow time for coverage planning and securing volunteers to stay over if needed.
• Conduct routine walk-throughs of the floor to identify and address any irregularities, such as misplaced equipment or missing PPE in isolation rooms. Report findings to the Wellness Coordinator to ensure timely resolution and compliance with safety standards. Motivate the other caregivers to keep moral strong on the floor.
• Provide leadership and guidance to the care team in response to resident care or behavioral concerns, ensuring appropriate intervention, documentation, and communication with relevant departments.
• Demonstrate exemplary care practices and positive resident interactions to model expected standards for the caregiving team.
• All other duties as assigned.

Supervisory Responsibilities
• The incumbent in this position will have no direct reports.

Education/Experience/Credentials
• Minimum of a high school diploma or equivalent.
• Minimum of 2 years’ experience in resident care or similar setting.
• Minimum of 1 year experience in a lead role.
• At least 21 years of age.
• Required Health Screening, TB Clearance, and Criminal Record Clearance.
• Any special licenses, certifications, and certificates (First Aid/ CPR Certified)
• Special skills needed to meet client needs [California Code of Regulations Sections: 85065(f)/87411(g)]
• Certified nursing assistant certification a plus.
• Able to speak, read, write, and understand the English language fluently.
• Ability to work additional hours which could include evenings and weekends.

• Familiarity with relevant healthcare regulations and compliance requirements for assisted living facilities.
• Knowledge of designing, implementing, and evaluating wellness programs tailored to the needs of elderly residents.
• Awareness of health and safety protocols to ensure the well-being of residents and staff.
• Understanding emergency protocols and procedures to handle crisis situations effectively.
• Knowledge of medication administration, storage, and adherence practices
• Ability to identify challenges and find practical solutions to enhance resident well-being and program effectiveness.
• Ability in utilizing software and technology for program management, documentation, and communication.
• Being flexible and adaptable in a dynamic healthcare environment to meet changing resident needs.
• Ability to Conduct wellness assessments and individualized care plans for residents.
• Working effectively with interdisciplinary teams, including nurses, therapists, and administrators.

GENERAL SUMMARY
