Salary Range: $114,296.00 to $148,584.80

This is a grant-funded position. Registered Nurse Case Manager/Clinical Diabetes Educator is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinical setting. Education is provided via telephonic communication, video teleconferencing, and in-person education sessions. This at times does require village travel. The educator will be responsible for ordering labs for patients according to hospital policies. The educator will be responsible for documenting education in electronic health records. This position will assist in the planning, coordinating, and providing of diabetes-related health promotion activities in the Delta Region. Patients and activities are accessed via multiple village trips per month. The educator will work closely with the village clinic and hospital staff to promote diabetes standards of care and inform them of new advances in diabetes care. The educator assists the Diabetes Coordinator with the maintenance of the Diabetes Registry and grant requirements. The educator works closely with the Registered Dietitian Clinical Diabetes Educator and Diabetes Pharmacist to ensure comprehensive care of clients. Educator participates in all program outreach activities such as monthly Diabetes Support Groups and radio shows, media campaigns, and special events. The educator participates in department planning and goal setting to achieve excellence in patient care and partnership with our tribal leaders. Educator believes in and promotes Napartet and all corporate endeavors. Performs other duties as assigned or required.
Experience/Education/Qualification
Experience:
• New graduates (with relevant licenses) may be considered.
• Two (2) years of experience in a clinical health setting is preferred.
• Demonstrated experience collaborating with a variety of disciplines for project completion.
• Background in Diabetes Education preferred.
Education
• Must have completed a program of education in nursing from an NLN-approved institution.
• Minimum Associate's Degree in Nursing is required.
• BSN or MSN preferred.
Licenses, Certifications, And/or Registration
• Valid license in good standing in the State of Alaska as a Registered Professional Nurse.
• Must obtain valid CDCES (Certified Diabetes Care and Education Specialist) certification within 2 years.
• Preferred Basic Life Support (BLS) certification.
COVID: All Travelers are required to show proof of completed COVID Vaccinations.
Knowledge/Skills/Abilities
• Work in a typical office, hospital, and village clinic setting.
• Comprehensive knowledge of PCs.
• Ability to operate diabetes care medical equipment such as glucometer, ht./weight scale, etc.
• Requires travel to villages via small plane/sub-arctic conditions, moderate lifting of equipment, and occasional long working hours.
• Strong knowledge base in the fields of diabetes, cardiovascular disease, and public health.
• Cross-cultural communication skills are essential.
• Bi-lingual (Yup’ik/English) preferred.
