About the Role:

Northern Adelaide Local Health Network (NALHN) is seeking an experienced and motivated Occupational Therapist to join our dynamic and supportive team. This is an exciting opportunity to contribute to high‑quality, person‑centred care for older adults across our acute services.

As the Allied Health Acute Aged Care Coordinator, you will provide expert clinical leadership and consultancy. You will play a pivotal role in facilitating integrated assessment, care planning, and timely discharge for eligible aged-care patients.

In this position, you will work collaboratively with multidisciplinary teams to identify older consumers who would benefit from comprehensive care planning, including behavioural support, mobility strategies and diversional programs. You will act as a NALHN specialist in aged care, dementia, and behaviours of concern, fostering strong working relationships with community partners, non-government organisations, and the Department for Health and Wellbeing.

A key component of this role involves monitoring and escalating system issues affecting patient flow and discharge timelines. You will support inpatients with complex needs through time‑limited case coordination, comprehensive assessment, and tailored strategies that promote safe, sustainable discharge and prevent unnecessary hospital representation.

To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.

To be successful in this role you will be an experienced clinician with deep expertise in dementia care, aged care, and multidisciplinary collaboration. You bring a balanced leadership style, strong communication skills, and a passion for delivering safe, high‑quality, person‑centred care. Key skills, experience and knowledge you possess include:

extensive experience delivering services to older adults, particularly those living with dementia
proven ability to provide comprehensive case coordination, behaviour support planning and transition planning
demonstrated high‑level communication skills, including professional consultation and advocacy across agencies
strong leadership capability
highly developed organisational skills, with the ability to manage competing priorities in a fast‑paced, changing environment
ability to build effective multidisciplinary relationships and foster teamwork across diverse disciplines
experience working with families, carers and community providers to improve outcomes for older adults
proven ability to initiate, plan, implement and monitor projects and service innovations
calm, objective and logical approach to problem‑solving and decision‑making
