Key Responsibilities:

Deliver high-quality patient education and support on various health topics under the guidance of Allied Health Professionals.

Collaborate with the Social Worker to address challenges preventing patients from attending appointments or adhering to treatment plans.

Ensure accurate maintenance of therapy-related databases and patient records under ******** direction.

Promote an integrated team culture responsive to consumer needs and assist in its development.

Offer practical assistance to patients and families where necessary, such as accessing transportation, accommodation, toiletries, clothing, etc.

Requirements: The ideal candidate will have demonstrated ability to work effectively as part of a multidisciplinary team and under remote supervision. Strong time management, organisational, and communication skills are essential, alongside a Certificate 4 in Allied Health Assistance or equivalent experience in a hospital or healthcare environment.
