Position: LVN Medical Coordinator
Exact Location: 2900 Louisiana Steet, Houston TX 77006
Duration: FTE/Perm
Working Hours:
Shift 1: 7am-3pm or,
Shift 2: 10am-7pm
Interview Times: Thursday: 8-10am; Friday: 9:30-1:30, 2-5pm
1st Round: Virtual Teams w/ Lead RN
2nd Round: In-Person w/ HR Rep, Shelter Manager, and APD
Must Haves:
• Bachelor’s degree in nursing
• LPN, LVN, RN, or BSN
• 1 year or experience in public health or healthcare setting w/ experience in care coordination, health administration, or direct clinical care setting
• Bilingual, English and Spanish
• Valid Texas Driver’s License
Day-to-day:
• Support 17 minors across St. Michael’s boys & girls shelter
• Provide over the phone and in person medical guidance to minors i.e. wrap and ice sprained ankle, take ibuprofen for pain, etc.
• Maintain accurate and updated documentation regarding biographical data, admission assessment, medical progress notes, Medication List, Medication Log, MARs, TARs, Sickness/Allergy Forms, Medical instructions/recommendations, medical summaries, Safety plans, SIRs, IRs
• Provide quality care and supervision to clients in accordance with ORR and TDFPS rules, program philosophy and procedure and client’s rights
• Maintain the ratio and close supervision through all activities
• Must report immediately to the supervisor any doubts, questions, medical concerns, or procedures regarding client’s health
• Ensure client’s information and medical files for are safe and kept confidential
• Effectively communicate information pertaining to the health clients according to the program policies and procedures to ensure quality services and client safety
