Job Summary:
As a Care Manager at Life Circle Health Services, you will play a pivotal role in providing exceptional care to our senior clients. Your primary responsibility will be to efficiently coordinate between clients and our home healthcare professionals, ensuring the delivery of top-notch care.
Key Responsibilities:
1. Placement and Deployment of Home Health Workers: Ensure suitable assignment and replacement of home health workers to meet the needs of our senior clients.
2. Gathering and Analysing Feedback: Collect and assess feedback from clients and families to continuously improve service quality.
3. Patient Care Plan Development and Updates: Collaborate with healthcare professionals to establish and maintain personalized care plans for each patient.
4. Assessment, Enrolment, and Patient Visits: Conduct assessments, facilitate client enrolments, and perform regular patient visits to monitor progress and address concerns.
5. Patient-Client Relationship Management: Build robust relationships with clients and families to provide emotional support and meet their needs effectively.
6. Caregiver Relationship Management: Foster positive relationships with our healthcare workers to enhance teamwork and service quality.
7. Payment Collection: Timely and accurate collection of payments from clients following company policies.
Special Requirements: Willingness to conduct home visits to patients. Company also provides mobile ambulance or self-driven cars to eligible Care Managers.
Qualifications:
- Educational Qualification: GNM/B.Sc Nursing/BMLT or any other paramedical qualification.
- Work Experience: More than 2 years. Preference given to candidates with management experience.
