Key Responsibilities
• Serve as a critical liaison between patients and healthcare providers.
• Manage a dedicated patient caseload, conducting thorough monthly follow-ups as their personal “Health Coach.”
• Address various topics in conversations, including medication adherence, symptom management, and individual support needs.
• Identify and help resolve barriers, guiding patients in achieving their health objectives set with their medical practitioners.
• Accurately verify and document patient details such as demographics, medical histories, medications, and conditions to create comprehensive clinical summaries utilizing certified software.
• Develop tailored care plans that consider the holistic needs of each patient, encompassing physical, mental, cognitive, and psychosocial factors.
• Act as a compassionate resource for patients, facilitating access to urgent chronic care when necessary, ensuring seamless continuity of care.
Required Skills
• Valid certification as a CMA, CCMA, or RMA.
• Proficient in HIPAA regulations and maintaining patient confidentiality.
• Exceptional communication skills, capable of managing challenging dialogues with professionalism.
• Fluency in Korean is essential.
Qualifications
• A minimum of one year of face-to-face healthcare experience is required.
• Preferred background in phone-based healthcare communication.
