A company is looking for a Per Diem Care Navigator.
Key Responsibilities
Process referrals to secondary care and maintain directories of specialists and community services
Manage communication and documentation for referrals to ensure continuity of care
Assist in training stakeholders on the external referral process and handle prior authorizations as needed
Required Qualifications
At least 3 years of applicable experience, preferably in a Primary Care setting
High school diploma / GED required
Proficient in Google spreadsheets and / or Excel, database management, and data tracking
