About the role:

The Care Partner (Clinical) will carry a reduced direct case load of care participants whilst also providing clinical oversight and input to care planning across the HCP Service.

Providing comprehensive care planning, brokering services, management of budgets, ensuring that care aligns with participant needs and preferences for participants assigned as care partner.

Providing clinical input to Care Plan development and review, conducting clinical assessments using standardised tools and contribute clinical expertise to multidisciplinary care planning and review

Identifying, sourcing and maintaining and supporting the team in applying appropriate clinical protocols.

Partake in an on call roster, where service delivery is provided between the hours of 5.30 pm – +pm Monday through Friday and 10 am to 4 pm Saturday and Sunday and Public holidays.

What you will bring to the team:

Qualification as a Registered as a Division 1 or Division 2 Nurse under the Australian Health Practitioner Regulation Agency (Nursing and Midwifery Board of Australia) or Member of Australian College of Mental Health Nurses

Extensive experience as a Care Manager, with in-depth knowledge and understanding of the Aged Care Quality Standards.
