About the Role:
As a Care Partner with Pearl Home Care, you’ll be responsible for coordinating personalized care plans, advocating for the needs of your participants, and ensuring they are supported to live independently and with dignity. You’ll be working closely with clients and their families, as well as a dedicated multidisciplinary team, to deliver high-quality care across the Liverpool-Wollondilly region.
Your role will include:
Case Management & Care Coordination: Develop and deliver individualized care plans, conduct assessments, and ensure seamless service delivery across home care and allied health services.
Client Advocacy: Be a trusted guide, coach, and support to participants and their families, helping them navigate services and setting goals that enhance their quality of life.
Participant Enablement: Empower clients to regain independence through enablement and reablement practices, while promoting autonomy, dignity, and safety.
Collaboration: Work closely with our nursing team, allied health professionals, and other support staff to ensure the highest standard of care.
Documentation & Reporting: Maintain accurate records and report changes in participants’ needs to ensure continuous, adaptive care.
What We’re Looking For:
We are seeking someone with:
Experience: 2+ years of case management experience in aged care (Home Care Packages, CHSP, or similar), with sound computer skills to operate care software.
Skills: Strong communication, organizational, and planning skills to manage a diverse caseload and engage with clients, families, and service providers.
Qualities: Passion for providing compassionate, client-centered care, and the ability to work both independently and as part of a supportive team.
Certifications: Valid Australian driver’s license, Police Check, and eligibility for NDIS Worker Screening. Cert IV in Individual support or higher qualification.
