Work/life: Alternates every 3rd weekend
Under the supervision of the Coordinator, provides emergency services to youth and adults with mental health and substance use disorders
Provides screening, triage, and intervention for persons experiencing crisis related to behavioral health conditions
Facilitates referrals to inpatient services as needed
Works closely with the Hope Line facilitate plans to address patients needs
The scope and volume of duties as well as target populations served will vary based upon site/department needs and will be assigned by respective Program Coordinator
Provides crisis support and stabilization activities including de-escalating persons in crisis, assisting therapists with facilitating hospitalizations for clients when indicated, actively assisting with safety planning, and coordinating with treatment team and afterhours services as need
Renders all services within department guidelines and in compliance with rules/standards set forth by Medicaid/Medicare, HFAP, OhioMHAS, and CARF
Works in tandem with Hope Line to manage crisis needs across the 5-county system
Will provide virtual services across the 5-county region
Provides face-to-face interventions at the Erie County office and Firelands Regional Medical Center’s ED as needed
Utilizes evidence-based screening techniques to assess for suicide risk
Takes the lead in developing safety plans and crisis management plans as assigned
Involves team members, natural support persons, self-help resources, and other community resources to ensure safety and crisis management plans are tailored to the unique needs of each individual
Provides crisis follow up and outreach on assigned patients across the system
Works collaboratively with assigned case manager to help patients build skills for crisis management
May assist with outreaching patients who have been discharged from inpatient facilities in order to promote engagement in follow-up, outpatient care

What You Will Do

What You Will Need
• Associate’s degree or higher in social services related field preferred. Experience working with persons with mental health or substance use problems preferred.
• Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
• Ability to effectively understand and communicate in both written and oral form. Sensitive to the cultural needs of the individual and/or family served.
