Caring Approach is a dynamic, vibrant, supportive and caring organisation. We provide tailored in-home support and community-based care for the elderly, people with both physical and intellectual disabilities, and those recently discharged from hospital.
We are seeking a caring and professional individual to become part of the Caring Approach team to coordinate services for people living at home. Caring Approach currently has an opportunity for a dynamic individual to join the team as Client Liaison / Rostering Officer in our office based in Macquarie, A.C.T.
This is a fixed-term 9-month contract to cover maternity leave. There may be potential for extension or a permanent role; however, this is not guaranteed and will depend on organizational needs.
We have a great office culture and, due to expansion, are looking for a dynamic individual with previous experience, strong administration skills, and the ability to engage with a variety of personality types to join a fun office environment in a rewarding role.
General duties include:
Rostering, coordinating and assisting in the service management of NDIS Participants and aged care consumers who have engaged Caring Approach
Meeting and engaging with new clients to explain what NDIS and Aged Care services Caring Approach can provide
Developing new service agreements and gathering required intake information and documents to ensure safe and person-centred service delivery
Liaising with clients and staff to coordinate services in line with individual needs, funding, and Caring Approach guidelines
Developing care plans and service duties
Maintaining client files
Resolving roster issues and reporting incidents through Caring Approach’s after-hours service and incident reporting process
Availability to be on call after hours when required
To be successful in this role you will need:
An understanding of coordinating and delivering care in a person-centred way
Experience in rostering and scheduling of services
Flexibility in your approach and willingness to assist team members across program areas
A thorough understanding of NDIS and the Aged Care system
Understanding of the SCHADS Award and rostering principles
Experience coordinating services for multiple clients in a community setting
Strong administrative and time management skills
Familiarity with Microsoft applications and the CareMaster rostering platform or willingness to learn
Strong problem-solving and prioritisation skills
Ability to engage with diverse clients, including those with complex and mental health needs
Ability to identify staff training needs
Ability to read, interpret and develop service information
Understanding of NDIS Quality & Safeguards Commission and Aged Care Quality & Safety Commission frameworks
Ability to manage and escalate complaints and feedback
Appropriate visa to work in Australia
Current Police Check (within 12 months) or willingness to obtain
Working With Children Check (WWCC)
NDIS Worker Check (NDISWC)
Minimum 12 months’ experience in a similar role
Current First Aid/CPR certificate
Driver’s licence (travel required)
Completion of NDIS Worker Module (or willingness to complete)
Completion of COVID-19 e-learning modules
Understanding of after-hours support requirements
Up-to-date with Caring Approach vaccination requirements
Desirable:
Experience supporting individuals to increase independence in a community setting
Background in nursing or related discipline
Ability to speak a second language
Ability to guide frontline staff in implementing care plans and support strategies
