Clinic Nurse (RN) and Care Coordinator (RN)

As an integral member of the ATSICHS Brisbane team, these positions will play a vital role in shaping and achieving ATSICHS's vision and goals within their scope. They will provide frontline nursing services in line with the IUIH Model of Care and contribute to the comprehensive health support for clients, aiming to improve health outcomes for Aboriginal and Torres Strait Islander people.

About the Role

Duties:

Work within a legal and ethical framework.

Provide effective first point of medical engagement including, triage assessments, taking of medical history/detail, assignment of patient and completion of preliminary procedures.

Accurately document client files with treatment and medication information as per care plan.

Ensure maintenance and confidentiality of patient health records.

Maintain recall and follow up register for clients as per recall policy.

Actively participate in planning and goal setting with the team to achieve team incentive plan outcomes.

Support the IUIH Model of care and demonstrate a commitment to integrated clinical service provision.

Contribute to maintaining a high level of infection control in the clinic as per RACGP Infection Control Guidelines 5th edition and ATSICHS Infection Control Policy.

Ensure the provision of a high level of vaccination practice in the clinic as per Australian Immunisation Handbook.

Operational quality improvement and the use of PDSA cycles to improve everyday operations at a local level as presented in regular staff meetings.

Work to foster productive working relationships with visiting service providers where necessary.

Promote and assist clients with complex care to a model of self-care.

Establish and maintain relationships with key external stakeholders conducive to holistic health and social support needs.

Conduct Home and Community visits in line with relevant ATSICHS Home Visit Policy and where appropriate.

Recognise when to advocate for clients to ensure appropriate access to transport, specialist/hospital appointments.

In consultation with GP and client, monitor, review, revise and action original GP Management Plan on a quarterly cycle (732) to determine patient chronic health status.

Follow all safety procedures and contribute to a safe work environment.

Perform service delivery within personal ability, observing universal precautions and follow standing orders/guidelines as per clinical policy and procedures.

Ensure maintenance and confidentiality of patient health records.

Maintain recall and follow up register for clients as per recall policy.

Demonstrated commitment to ongoing training and regular updates in the quality control system.

- Starting annual salary of $81,000 (depending on qualifications and experience).

- 11.5% Super + Not for Profit Salary Packaging (Increases your take home pay).

Desired Skills and Experience

To be successful in this role, you will need to have the following skills and experience:

- Bachelor degree in nursing or higher.

- Full un-restricted AHPRA registration.

- Significant experience working with Aboriginal and Torres Strait Islander clients.

- Demonstrated ability to meet targets and performance outcomes.

- Ability to support a continuous improvement model for achieving outcomes.

- Decisive listening, influencing and persuasive skills.

We also prefer candidates with the following experience:

- Minimum two (2) years experience in a primary health setting.

- Immunisation endorsement and experience.

- Previous experience in a multidisciplinary health team.

- Previous experience in chronic disease management.
