Responsibilities

The Clinical Care Coordinator participates as an integral member of the multidisciplinary care team at PMGA. The Clinical Care Coordinator collaborates with and outreaches to the outside service providers to provide seamless transitions of care, and provides patient education while assisting with access to applicable resources.

Required qualifications:
• Associate's Degree in Healthcare Related Or In lieu of Associate's degree: 2 years of experience in an outpatient setting OR related coursework/training and 1 year of experience in an outpatient setting.
• Within 30 days of hire: National Provider BLS - American Heart Association

Preferred qualifications:
• Bachelor's Degree in Healthcare Related
• 2 years of Care coordination experience.
• Experience in an outpatient setting.
