Permanent Part time - 32 hours/week
To deliver health services to clients in the Canberra area.
Competitive base rate plus superannuation with eligibility to salary packaging (up to $18.5K tax free)
Ability to work from home with some travelling required for face to face assessments & service promotions with clients, as per business requirements
Travel reimbursement payment for use of own private vehicle.

Reporting to the Senior Manager - Health Services, the Senior Health Coach will work collaboratively to improve the quality of clients care through facilitating the efficient use of resources thereby enhancing quality, cost-effective outcomes.
At integratedliving, our Senior Health Coaches work collaboratively with other health professionals to assess, plan and implement restorative or reablement activities for our clients. The focus of this role is to enable prioritization and individualization of health management through promotion of health literacy and empowerment of clients and their families/carers through coaching, education and use of assistive technologies, to achieve and maintain optimum level of health and well-being.
You will be visiting our clients in their homes to assess, plan, implement and coordinate their Short-Term Restorative Care program in the Canberra area.
You will work within Community Care Standards and EQuIP Quality Framework
You will monitor the Health Management of clients over an 8 week program
You will empower and support clients to achieve their optimal levels of functioning independence and self-care
You will educate clients, carers and their families around the areas of wellness and reablement including ongoing Chronic Disease Management
You will enjoy working independently to promote the service and to seek referral of new clients to ensure full occupancy of the program
Building and maintain relationships with local health professionals and services to ensure a high quality of care is maintained throughout the program
What you need to be considered for the role ?
Degree qualification in nursing or allied health.
Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA)
3+ years of clinical or health management experience in Nursing or Allied Health
Experience within a health or community aged care setting
Ongoing commitment to Continued Professional Development and Education
Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion
Demonstrated understanding of evidenced based care
Ability to conduct Case Management and Health assessments
Strong interpersonal skills, problem solving, analytical and reporting skills
Ability to work as part of a wider team and autonomously in a remote model of working
Ability to organise work in a systematic, timely and accurate manner
Well developed IT skills and demonstrated competence in delivering services via virtual health technologies and a willingness to learn and adopt new technologies
Current driver's license and ability to travel within the region on a regular basis
Satisfactory Police Check and Current CPR
