The Role

We are seeking an experienced Registered Nurse to join our clinical community nursing programme service. As a member of our team, you will provide high quality nursing services to people living in the community. Your role will involve working closely with our established team of Nurses, Coordinators, Support Workers and other Allied Health professionals to deliver care to clients in their own homes.

Responsibilities

Your key responsibilities will include:

Providing a broad range of care to clients in their own homes, including palliative care, wound care, medication management and diabetes management.

Working collaboratively with our multidisciplinary team to develop and implement individualised care plans.
">
Conducting regular assessments and evaluations to ensure the highest level of care is being provided.

Developing and maintaining effective relationships with clients, families and carers.
">

Requirements

To be successful in this role, you will need:

A current registration as a Registered Nurse.
">
At least 1 year's experience as a Registered Nurse.
">
A restricted or full driver's license.
">
Strong communication and organisational skills.
