About the role
This is a permanent part-time position (~0.5 FTE, approximately 4 hours per day) and is primarily work from home, with flexibility in how you structure your day.
You’ll take clinical ownership of a portfolio of clients, ensuring care is safe, consistent, and responsive as care needs evolve over time.
Day to day, you will:
Oversee clinical care for people living with complex and progressive conditions
Supervise and guide disability support workers, providing clinical direction and oversight
Conduct care assessments, develop care plans, and review these as needs change
Provide oversight for complex clinical supports, including PEG feeding, tracheostomy care, and respiratory support.
Coordinate safe hospital-to-home discharge pathways, working closely with hospital teams and allied health providers
Lead intake assessments and onboarding for new clients
Ensure care delivery aligns with NDIS practice standards and compliance requirements
Contribute to quality improvement and service development
In-person requirements: Some travel within metropolitan Melbourne is required for client assessments and occasional team meetings.
You’ll be supported by an experienced team of healthcare professionals, including clinicians with extensive expertise in complex disability care.
About you
You are a Registered Nurse (AHPRA) with experience in acute, community, or complex care settings.
You may come from ICU, Emergency, HDU, or community nursing — environments where you’ve developed strong clinical judgement, confidence managing risk, and the ability to make decisions independently.
You bring both clinical skills and person-centred care. You understand the importance of building trust with clients and families, particularly when supporting people with complex or progressive conditions.
You’ll be well suited to this role if you:
Have solid post-registration experience, including exposure to complex or high-acuity clinical environments
Have experience with complex clinical supports
Are confident working independently in a leadership role.
Can communicate effectively and compassionately with clients, families, and multidisciplinary teams
Experience with the NDIS framework is desirable, but not essential. We will support the right person to build this knowledge.
This role requires strong clinical judgement, independence, and the ability to manage complexity with confidence.
Salary and benefits
$90,000 – $95,000 full-time equivalent (pro rata), plus superannuation
