Job description:
Provide clinical leadership in the development, provision, and evaluation of Hospital@home service that ensures optimal client/family/carer and community outcomes.
The role:
In accordance with relevant Agency principles and policies, legal requirements and professional competencies, the Clinical Nurse Specialist (CNS) – Hospital@home (formally Community Rapid Response Service and Hospital in the Home) will:
Provide clinical leadership in the development, provision, and evaluation of Hospital@home service that ensures optimal client/family/carer and community outcomes.
Provide nursing expertise, care coordination, nursing management and education in the field of acute and sub-acute care to patients referred to the Hospital@home service.
The Hospital@home service, catering to public and private patients of Southern Tasmania, plays a crucial role in our community. It provides hospital avoidance interventions and acute and sub-acute care for adults, ensuring safe and effective care in the community setting.
The service operates across multiple sites throughout Southern Tasmania. Hospital@home satellite sites are currently located in Cambridge, Glenorchy, and Risdon Vale.
