About the Role

We have an opportunity for a senior nurse to join the Clinical Nurse Specialist, ElderHealth team. Our team provides community focused, culturally responsive, integrated health care for kaumatua and older adults in our region.

The role encompasses both the community ElderHealth specialist nursing service, and involvement in the transdisciplinary OPAL (Older Person's Assessment and Liaison) Community Service.

We are seeking to recruit a senior nurse who can ensure the delivery of clinical and cultural best practice across the region. This role will be based in Horowhenua but supported by a team of senior nurses who engage in comprehensive assessments and support, within the care continuum in a variety of care settings including people's homes, throughout Te Pae Hauora o Ruahine o Tararua, Midcentral region.

About you

Excellent knowledge of complex health conditions, frailty, dementia, and elder health
Can provide advanced nursing expertise, clinical assessment and co-ordination of support
Demonstrate a high level of critical thinking, solution strengths based and focused problem solving
Enable and empower clients and their family/whānau to participate in whānau centred engagement
Have confidence and passion to work collaboratively with multidisciplinary and transdisciplinary teams, community organisations and cultural groups to support an inclusive, innovative, culturally responsive and respectful service
Proven leadership, and ability to engage with tangata whenua and iwi providers is an advantage for this role
A postgraduate qualification with prescribing qualifications or willingness to obtain is a pre-requisite
