The Role:

We are seeking an experienced and passionate Clinical Nurse Specialist (CNS) with a thorough understanding of Renal to join our Pre-Dialysis team. This senior nursing role provides expert clinical leadership and specialist care to patients living with chronic kidney disease (CKD) in the community and across the continuum of care.

We are the largest Renal Service in New Zealand and steadily growing, which means we need experienced renal nurses to meet our patients needs. Working alongside other Pre-Dialysis CNS you will be managing your own case load of patients with chronic kidney disease; you will be responsible for their ongoing education and monitoring as well as supporting people to make decisions regarding dialysis - modality, and lifestyle changes.

You will need to demonstrate your ability to work towards and achieve deadlines while managing a varying workload. Adept at problem solving, you will demonstrate a flexible and innovative approach.

Key Responsibilities:

Clinical Leadership & Expertise
Provide advanced specialist nursing assessment, case management, and clinical leadership for high-risk CKD patients
Lead nurse-led CKD outpatient and pre-dialysis clinics in collaboration with the multidisciplinary team
Triage and manage complex referrals, developing and implementing individualised care plans
Ensure continuity and coordination of care across primary, secondary, and tertiary services
Professional Practice & Service Development
Contribute to the development, implementation, and evaluation of renal and CKD frameworks, policies, and clinical guidelines
Promote and embed evidence-based practice and quality improvement initiatives
Participate in audit, research, and evaluation activities to enhance clinical outcomes
Provide expert input into service planning, innovation, and equipment evaluation
Education, Coaching & Mentorship
Educate and support nursing staff and the wider MDT on CKD management and available resources
Provide coaching, mentoring, and clinical supervision to support professional development
Contribute to local and national education initiatives and guideline development
Cultural Safety & Equity
Deliver culturally safe, patient- and whaanau-centred care
Demonstrate commitment to Te Tiriti o Waitangi and improving outcomes for Māori
Work respectfully with diverse communities, acknowledging cultural beliefs and practices

About You:

NZ Registered Nurse with a current Annual Practicing Certificate
Minimum 5 years recent renal nursing experience in NZ (post registration)
Experience in case management
Current PDRP demonstrating senior nurse attributes
Relevant post graduate qualification completed or prepared to undertake
