About the role

Community healthcare provision is key to supporting our health system and community. Southern is going through exciting, innovative changes to provide care to our community.

The Community Clinical Coordinator role will support both inpatient and community teams to coordinate efficient, effective and safe services, within available resources. This role is varied, with its main driver being patient flow, it will coordinate and work with teams to get people home living quality lives.

The initial purpose will be to embed clear systems to ensure patient flow across the hospital and community for ACC contracts. Coordination of interim care into ARC facilities and relationship building with interim care facilities will be key in this role.

You will work alongside:

community teams
inpatient teams
ARC facilities
primary care agencies
unit managers
other clinical coordinators.
The role has the potential to grow and change with our change in modes of care between the hospital and community - the opportunities are endless

This role provides leadership for the coordination of clinical service delivery, clinical leadership and direction to staff across community.

In this role you will:

provide clinical leadership
monitor and mitigate risk
coordinate ACC contracts
actively engage in quality initiatives
manage the day-to-day functioning of the service to ensure delivery of safe patient focused care
support staff education.
This position does not encompass budget responsibility but has delegation of authority for management of staff within the community team.

have an excellent knowledge of adult teaching and learning principles
demonstrate strong clinical decision-making skills
have excellent communication and engagement skills
have a genuine interest in people and intrigue in different models of care
have advanced communication techniques such as conflict resolution, diffusion and mediation skills.
