Responsibilities

Conduct initial and ongoing assessments for assigned patients, developing individualized care plans as needed.

Provide treatment to assigned patients within your scope of practice, meeting program and department productivity and quality standards.

Work effectively as a member of our multidisciplinary treatment team, communicating with providers, physicians, case managers, and other stakeholders.

Maintain accurate documentation, complying with department standards and patients' insurance requirements.
