Your key responsibilities will include:
Navigation Support: Build strong relationships with agencies and community partners to help clients access a range of services, including NGOs, community organisations, and government social development services.
Community Alignment: Work within a Pacific model of care, reflecting our core values and beliefs to enhance the overall well-being of Pasifika people.
Collaborative Approach: Partner with General Practice and the LagiOla team—including Psychiatrists, Psychologists, Mind Health Practitioners, Health Coaches, and other Fono services—to deliver integrated, holistic support.About you:
