The post holder will manage autonomously as part of a highly motivated team of Case Managers providing a high standard of care, utilising holistic skills in the assessment, planning and implementing pro-active care needs to a population with Long Term Conditions in the community settings. The post holder will work in collaboration with Primary and Secondary care clinicians, social care and mental health colleagues, to ensure care is patient centred and appropriate to their assessed needs. The post holder will be a champion for Trafford Case Management and be able to demonstrate their clinical and professional skills and competence in team work, communication, delegation, care planning / co-ordination of individual's needs, medicine management and health and case management. The role requires the post holder to travel between the 4 neighbourhood sites and to patients homes., Trafford Case Management (TCM) is an exciting new service being developed in Trafford in order to enhance the management of patients with complex care needs in conjunction with the multidisciplinary team., Working across the Trafford Localities the Case Manager will undertake the care of a defined caseload of patients working closely with and based within one of our four integrated neighbourhood teams .You will have the relevant experience and skills of a case manager to undertake this role., To find out more about the key responsibilities and the specific skills and experience youll need, take a look at the Job Description & Person Specification attachments under the Supporting Documents heading.
