We are hiring a Case Manager / Community Support Specialist to assist individuals and families with accessing essential services and navigating available programs.
This role is ideal for candidates with a background in social services, case management, or community outreach.
Key Responsibilities
• Conduct intake assessments and identify client needs
• Connect clients with community resources and support services
• Maintain detailed case documentation and follow-ups
• Provide ongoing support and guidance to clients
• Collaborate with internal teams and external agencies
Qualifications
• Bilingual Spanish / English required
• 1+ years of case management, social services, or related experience
• Bachelors Degree
• High Volume Calls
• Strong interpersonal and organizational skills
• Experience working with vulnerable populations preferred
Preferred
• Experience with Medicaid / Medicare / public assistance programs
• Background in nonprofit, healthcare, or community-based work
