The Role:
As our Community Worker, you will deliver community-based support services and implement programs aimed at improving the social, emotional, and practical wellbeing of individuals and families.
Responsibilities:
· Assess clients’ needs and identify appropriate support services in line with individual circumstances
· Develop, implement, and review individual support plans in collaboration with clients and relevant stakeholders
· Provide information, guidance, and referrals to community services, including health, housing, and social support services
· Support clients in accessing community resources, programs, and government services
· Liaise with healthcare providers, social workers, and other service agencies to coordinate care and support outcomes
· Advocate on behalf of clients to ensure access to appropriate services and protection of their rights
· Monitor and evaluate client progress, adjusting support strategies where required
· Maintain accurate and up-to-date case notes, reports, and documentation
· Facilitate group programs and community engagement activities to promote social inclusion
· Assist in crisis intervention and provide emotional support where required
· Ensure all services comply with organisational policies, NDIS guidelines, and relevant legislation
· Participate in team meetings, case reviews, and continuous professional development.
Skills & Attributes:
· Relevant qualification
· Minimum 1 year of relevant experience
· Strong interpersonal and communication skills
· Ability to work independently and as part of a team
· Demonstrated empathy, patience, and commitment to client wellbeing
· Knowledge of NDIS framework and person-centred care principles (preferred)
