About the Role:

GNG Healthcare Group is seeking a compassionate and experienced Community Worker to facilitate community development initiatives and collective solutions that address issues, needs, and problems associated with recreational, health, housing, employment, and other welfare matters within the communities we serve. This role is crucial in helping individuals maintain independence and improve their quality of life by connecting them with essential services and resources, aligning with GNG Healthcare's commitment to personalized and accessible care. The Community Worker will focus on advocacy, resource linkage, and program development, operating at a skill level commensurate with a Bachelor's degree or equivalent.

As a Community Worker, your primary duties will include:

Assess community needs and resources, focusing on the health, welfare, housing, employment, and recreational needs of GNG Healthcare's clients and the broader community.
Liaise and collaborate with diverse community groups, welfare agencies, government bodies, and private businesses to identify systemic community issues and promote awareness of available resources and services.
Plan, develop, and implement educational, training, and support programs that empower individuals and groups, enhancing their quality of life and promoting independence and well-being.
Interview clients to understand the nature and extent of their difficulties, and provide appropriate support and information, coordinating and facilitating their access to healthcare services, social activities, and community resources.
Monitor and report on the progress of clients within the community context, ensuring effective service delivery and advocating for improved outcomes.
Refer clients to relevant agencies and services for additional help, ensuring comprehensive and integrated care pathways that complement the personalised care provided by GNG Healthcare.
Prepare detailed submissions for funding and resources, as well as comprehensive reports for governmental bodies and other relevant agencies, contributing to community development efforts.
Collaborate with families, case managers, case workers, support coordinators, and healthcare professionals to develop and implement holistic care plans that address broader community needs.
Ensure all activities comply with organizational policies and relevant legislation, including the NDIS scheme processes and person-centered planning and supports.
Maintain accurate records and documentation of client interactions and community progress.

Qualifications and Experience:

Bachelor's degree or equivalent in healthcare coordination or community services.
Minimum of 1 year experience in community services, focusing on facilitating community development, resource coordination, and addressing welfare matters.
Current and Valid Western Australia Driver's Licence (other Australian States Driver's Licences accepted).
Current First Aid and CPR certification (or willingness to obtain).
NDIS Worker Screening Check (or willingness to obtain).
Strong communication and interpersonal skills, with the ability to engage effectively with diverse stakeholders and clients.
Ability to work independently and as part of a multi-disciplinary team.
Desirable Additional Experience

Experience working with culturally and linguistically diverse (CALD) communities.
Training in mental health first aid or psychological support.
Background in case management, support coordination, or allied health services.
Familiarity with the NDIS scheme processes and person-centered planning and supports.
