About the program
The Medical Services Program delivers primary health care through GP and specialist services, supporting a diverse community through accessible and culturally safe care. The program works closely with Allied Health and other internal services to provide holistic, coordinated support to clients with complex health needs.

About the role
The Diabetes Nurse Educator provides culturally safe, evidence-based assessment, education and support to clients to promote diabetes prevention and self-management. Working closely with GPs, Allied Health and community services, this role supports coordinated care for clients with complex and chronic health needs.

Key responsibilities include:

Delivering high quality diabetes education to individuals and groups

Supporting clients with diabetes management, including insulin stabilisation

Coordinating care and working collaboratively across multidisciplinary teams

Providing clinical advice and support to GPs and other health professionals

Maintaining accurate clinical documentation and supporting quality improvement

Contributing to staff education and development

About you
You are a compassionate and skilled clinician who is passionate about improving health outcomes through education, empowerment and culturally responsive care.

Current AHPRA registration as a Registered Nurse (Division 1)

Graduate Certificate in Diabetes Education (or working towards)

Credentialled Diabetes Educator status (or working towards)

Demonstrated experience in diabetes education, including insulin stabilisation

Strong understanding of chronic disease self-management principles

Experience working with diverse communities and complex health needs

Ability to work collaboratively within a multidisciplinary team

Commitment to culturally safe, person-centred care
