Job Title: Diabetes Nurse Prescriber – Community-Based Tripod Partners are currently recruiting for a Band 7 Diabetes Nurse Prescriber to work with the South Sefton Diabetes Team. Key Responsibilities You will provide expert diabetes management, including prescribing medications, and offering specialist care to patients in both clinic settings and during home visits. Collaborate with the multi-disciplinary team to ensure comprehensive diabetes care and support for patients in the community. Initially, support and guidance will be provided via shadow shifts to ensure a smooth transition into the role.
