Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. Position Summary:
We have an exciting opportunity to join our team as a Counselor - Fulltime - Spanish Speaking Required.
In this role, the successful candidate The Counselor is responsible for providing case management support for families that have children under the age of 5. This includes assessing family needs, screening for program and benefit eligibility, assisting with benefit application and navigation of services, and providing psychoeducation. Excellent engagement skills and ability to work with individuals and families.
Job Responsibilities:
• Conducts outreach and educational workshops
• Screens clients and family members for social determinants of health
• Provides advocacy and assistance in navigating and accessing services
• Attends staff meetings and other appropriate meetings provided by the department and organization.
• Identifies and makes referrals to a variety of public and community agencies providing food, shelter, clothing, medical, educational, and other services.
• Manages caseload and meets productivity standards set by department.
• Documents all work in EPIC and other required platforms.
• Assists clients in applying for or renewing benefits.
• Provides information and education on topics such as parenting, nutrition, and developmental milestones
• Other duties as assigned by Family Support leadership.
Minimum Qualifications:
To qualify you must have a High School Diploma or equivalent required.
Bachelor's Degree preferred.
1+ years relevant experience working, ability to plan and manage,
Knowledge of public benefits
History of immigrant community engagement

• May provide input to the protocol design document; leads development of the protocol, including protocol amendments, Protocol Administrative Change Letters, and Dear Investigator Letters
• Represents the study team in internal protocol review governance submissions and interactions; partners with/supports the Global Development Lead in study and disease area strategy; may participate in protocol-related KOL and Health Authority interactions
