Disability Care Coordinator
About the Role
You will be working as a Support Worker, providing essential support and care to adults with intellectual disabilities.
As a member of our team, you will be responsible for delivering person-centered care, promoting independence, and enabling individuals to participate fully in their communities.
The successful candidate will possess excellent communication and interpersonal skills, with the ability to build rapport with people from diverse backgrounds.
Key Responsibilities:
Providing support and assistance with daily living tasks, such as personal care, meal preparation, and household chores.
Developing and implementing individualized plans to promote independence, social inclusion, and community participation.
Collaborating with multidisciplinary teams to ensure seamless service delivery and coordination.
Providing emotional support and guidance to individuals experiencing emotional overwhelm or challenging behaviors.
Maintaining accurate records and reports to ensure quality service delivery and compliance with regulatory requirements.
Requirements:
Diploma or Certificate in Health and Wellbeing or equivalent.
Experience working with adults with intellectual disabilities.
Excellent communication and interpersonal skills.
Able to work independently and as part of a team.
Flexibility to work varied shifts, including days, evenings, and sleepovers.
