We’re looking for dedicated and reliable Support Workers to join our compassionate team. In this role, you’ll deliver person-centred support to NDIS participants and older adults, helping them live independently and achieve their personal goals — both at home and in the community.
You will need to be flexible with travel, as our participants are located across Sydney.
Key Responsibilities
Work across weekdays, weekends, evenings, and occasional sleepover shifts.
Provide in-home and community-based support in line with each client’s service plan.
Assist clients in engaging in age-appropriate activities, employment, and community participation.
Support clients with outings, appointments, and social activities.
Ensure client safety, privacy, dignity, and confidentiality at all times.
Monitor client wellbeing and report any concerns to the Manager.
Maintain accurate and timely documentation, including client notes and communication records.
Collaborate with families, carers, case managers, and other professionals to support individual goals.
Follow policies on medication administration, workplace health and safety, and client rights.
Qualifications & Requirements
Certificate III in Individual Support, Disability, Aged Care, Community Services, or Mental Health (or similar).
Minimum 2 years of relevant experience in Disability Care and/or older adults.
Current National Police Check and Working with Children Check.
First Aid and CPR certificates.
NDIS Worker Orientation Module completed.
100 Points of ID.
Additional language skills are highly regarded and beneficial.
