Family Support Worker – Aged Care
Location: Perth WA
Position: Full-Time
Salary: $75,000 – $80,000 + Superannuation
On behalf of our clients St Louis Care at Home, an established aged care provider in Claremont, Perth is seeking an experienced and compassionate Family Support Worker to join their team. This position plays an important role in supporting elderly clients and their families through wellbeing support, care coordination, client advocacy, and community engagement initiatives.
The successful candidate will work collaboratively with care staff, families, allied health professionals, and external stakeholders to help deliver person-centred support services that enhance client wellbeing and quality of life.
Key Duties
Provide ongoing emotional and welfare support to elderly clients and their families
Assist with care coordination and communication between residents, families, and care teams
Support clients experiencing social isolation, behavioural concerns, or emotional challenges
Contribute to care planning and client wellbeing initiatives
Liaise with external health professionals and community support services
Maintain accurate case notes, reports, and client documentation
Support culturally appropriate engagement with residents and families
Promote a safe, respectful, and inclusive care environment
Participate in multidisciplinary team discussions regarding client wellbeing outcomes

Selection Criteria
Australian Diploma in Community Services, Individual Support, Welfare, Community Development, or an oversea equivalent
At least 1 year of full-time work experience in community services, disability support, or client welfare support
Strong interpersonal and communication skills
Ability to build positive relationships with elderly clients and families
Sound organisational and record-keeping abilities
Ability to work effectively within a team environment
English level of IELTS 5.0 and above, or equivalent.
