Job Description
Summary:
As a key member of the North Peel Family Health Team (NPFHT), the Foot Care Nurse (RN/RPN) will deliver foot care in accordance with NPFHT policies and professional practice standards, and according to regulations set out by the College of Nurses of Ontario. The RN/RPN will perform their duties and ensure that the needs of the patients are met.
The Foot Care Nurse’s primary role is to work in partnership with NPFHT Physicians, affiliated physicians and other allied health practitioners and support staff to perform patient care. According to The Nursing Act, the Nursing Scope of Practice Statement states that nursing is the promotion of health and the assessment of, the provision of, care for, and the treatment of, health conditions by supportive, preventive, therapeutic, palliative, and rehabilitative means in order to attain or maintain optimal function. (Nursing Act, 1991)
Accountability:
The Registered Nurse (RN) or Registered Practical Nurse (RPN) reports to, and is responsible to, the Executive Director or delegate. All clinical decision making outside the scope of the Registered Nurse or Registered Practical must be done in consultation with physicians of NPFHT, non-affiliated physician partners and/or NPFHT medical directives.
Roles and Responsibilities:
Assess patient health care needs (physical, emotional, psychological, and spiritual) to identify appropriate interventions.
Collect data through a therapeutic relationship with a patient.
Critical thinking and clinical judgment skills to provide safe, competent, and ethical foot care.
Adhere to infection prevention and control and reprocessing practices, guided by best practice guidelines, evidence informed literature and manufacturer’s instructions.
Apply evidence-informed principles when assessing, planning, implementing, and evaluating clients’ needs for foot care.
Practice within, and adhere to, best practice guidelines, nursing standards, established procedures and policies for foot care.
Maintenance and sterilization of foot care tools and other medical instruments.
Education/Advocacy:
Help patients to identify and use health resources, including those available in the community and from community agencies.
Involve patients in decisions about their own health.
Encourage patients to take action for their own health.
Initiate health education and other activities that assist, promote and support patients as they strive to achieve the highest possible level of health.
Foot care nurses are responsible and accountable for maintaining and improving their competence to practice.
Foot care nurses recognize when clients’ needs are beyond their professional scope of practice or personal level of competence and will refer to the appropriate professional.
Referrals/Collaboration:
Collaborate with members of the health care team to plan, implement, coordinate, and evaluate patient care in consultation with patients and their families.
Administration/Reporting:
Manage patient phone calls and messages.
Arrange for follow-up appointments, as necessary.
Collect accurate and comprehensive patient-related information and document in electronic medical record.
Manage and dispose of specimens and/or objects, and practice general safety procedures at all times.
Monitor supply levels and/or expiration of medical and clinic supplies
Interprofessional Collaboration:
Develop and hone collaborative community relationships to facilitate patient access to services.
Communicate ideas or concerns to the health care team. Participate in problem solving to enhance patient care.
Foot care nurses promote optimal health, collaborating as integral members of the nursing team.
Practice Environment:
Use of appropriate personal protective equipment with all patients.
Maintain clean, orderly, and well stocked inventory of medical supplies
Assist with other related duties as required.
Education/Experience:
Must possess a license in good standing with the College Nurses of Ontario as a Registered Nurse or Registered Practical Nurse with minimum one year foot care experience
Basic and/or advanced foot care certificate required
Must possess membership in good standing with RNAO or RPNAO
Skills/Abilities:
Demonstrated ability in utilizing the nursing process in planning, implementing, and evaluating patient care.
Knowledge and proficiency in current, evidence-based methods and practices of foot care delivery, with an emphasis on health promotion and risk reduction.
Organization, evaluation, time management, communication, and interpersonal skills.
Demonstrated ability in problem solving using sound judgment and critical thinking.
Ability to prioritize, manage time effectively and be flexible in a very active constantly changing work environment.
Proficiency in the use of computers and information technology is required, including the use of electronic medical records (EMR)
Excellent communication skills in English (spoken, written, reading and comprehension)
Pay: $28.00-$35.00 per hour
