THE ROLE
This position will focus on the development and implementation of health and education initiatives for children and young people in care services so that their diverse needs are better identified and responded to.
You will have responsibility for developing and supporting cross agency processes that ensure timely health and education assessments, support the coordination of children and young people's treatment and support packages as determined through the assessment process.
You will work in close consultation and partnership with Department of Health, Department of Families, Fairness and Housing central staff, the Department of Education and Training, Community Service Organisations, Aboriginal Community Controlled Organisations, local schools and education providers, and community health providers.

Skilled in planning and organising, gathering and analysing information, and report writing?
ACCOUNTABILITIES INCLUDE

Drive or actively support the establishment and practice of the local Education Supports for Children in Care (ESCC) service model and program/s in Outer Eastern Melbourne bringing together key education related services.
Maintain oversight of children identified as disengaged from education, in collaboration with LOOKOUT, ensuring they are linked into ESCC or other appropriate supports.
Identify and address escalating barriers that impact children and young people accessing or making the most of supports and working with stakeholders to address the issues.
