Job Summary
The Registered Nurse (RN) will provide high-quality, patient-centered care to individuals in their homes, as prescribed by physicians and in accordance with applicable laws, regulations, and agency policies. The ideal candidate will have excellent communication skills, a strong focus on patient safety, and the ability to work collaboratively with healthcare professionals to achieve optimal outcomes.
Responsibilities
Conduct comprehensive assessments of patients to determine primary focus of care and develop individualized plans to meet their unique needs.
Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure continuity of treatment.
Provide education to patients and their families on disease management, self-care techniques, and medication adherence.
Monitor patients' progress, identify areas for improvement, and adjust care plans as needed.
Requirements
CURRENT RN LICENSURE IN STATE OF LOUISIANA
CURRENT CPR CERTIFICATION
CURRENT DRIVERS LICENSE, VEHICLE INSURANCE, AND ACCESS TO A DEPENDABLE VEHICLE OR PUBLIC TRANSPORTATION
