NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.
The shift pattern for this post is Monday - Friday
As part of the heart failure team, you will work as an ANP and assist in coordinating the provision of seamless care, in conjunction with medical staff and to liaise with other members of the Multi-disciplinary Team to facilitate investigation, treatment, admission avoidance of heart failure patients as efficiently and effectively as possible.
You will provide a high quality, safe and supportive environment in order to care for heart failure patients within NHSGGC meeting the identified physical and psycho-social needs.
The post-holder will be expected to work across roles and geographical boundaries, providing complete clinical care for their patient as part of a multidisciplinary team by serving a wide patient base within the heart failure service.
They will:
Identify appropriate referrals
Support implementation of management plan under supervision of ANP
Initiate information to new referrals
Work under own initiative, manage time effectively, support ANPs.
Provide accurate and timely data entry/record keeping.
Participate in audit/research.
Flexibility regarding the remit and scope of the role is expected.
optimise medication in stable patients under supervision of ANP and within service guidelines.
Prioritise caseload with support of ANP if necessary.
Liaise with ANP colleagues/medical staff/GPs on regular basis regarding heart failure therapy.
Foster good relationships with a wide variety of departments and voluntary services
Acts as mentor / supervisor to nursing students
Participate in home visits
