Based in the Burwood area in Sydney's Inner West, our client is a long established, highly reputable community care organisation, servicing a local client base from culturally and linguistically diverse backgrounds.
Several exciting opportunities have arisen to join their team in the positions of HCP Care Advisor and NDIS Care Advisor.
The Role
Each of these roles will focus on conducting initial and ongoing client assessments within their homes, and delivering strategies, guidance and support to enhance their mobility, independence and wellbeing. This will include:
Provision of high-quality case management services for language-specific clients
Undertaking a comprehensive assessment of each client in collaboration with the client and their family or carer
Determining health and wellbeing maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain a quality of life
Coordinating the safe delivery of quality holistic care to the client and monitoring client progress
Engaging with allied health professionals and community supports to enhance client care delivery
Understanding client budgets and aligning services and supports with available funding
Coordinating services in line with the funding bodies' requirements, Social Justice and Access & Equity principles, WHS requirements and the organisation’s philosophy, mission, policies and procedures
Developing associated documents and reporting, as required
What You'll Need
In order to succeed in this role, you will require the following:
Current AHPRA Registration as a Div. 1 Registered Nurse or other clinical specialist
Experience in a case management role within a care focused service or setting
Demonstrated understanding of the needs and issues of older people or people living with disabilities, and their carers
Demonstrated experience in needs assessment and management of older people or people living with disabilities
Excellent written and verbal communication skills
Demonstrated ability to supervise and develop out of office staff and speak a relevant second language
Computer literacy
Valid National Police Check
Valid NSW Driver's Licence and own vehicle
Up to date COVID-1+ vaccination
Although not essential, relevant qualifications would be advantageous, as would the ability to speak a second language, and an understanding of the Home Care Package or NDIS funding and supports.
