We are looking for a case manager to join our aged care program.
POSITION SUMMARY
To ensure high quality experience of home care services to Home Care Package recipients based on the Values of Senior Helpers and the Aged Care Quality Standards (2019).
SERVICE OBJECTIVES
Provision of ethical and responsible services to Home Care Package recipients.
Effective and transparent management of Home Care Packages - financial, service, communication and collaboration and clinical.
To embody Senior Helpers’ Mission and Values through Home Care Package services.
LEVEL OF RESPONSIBILITY
The position is managerial and therefore is responsible to the Owners/Franchisee for all aspects of work performance. The position is subject to minimal guidance following induction training and the employee is expected to exercise leadership, initiative and judgement in the performance of roles as a representative of Senior Helpers.
SUPERVISION
The Case Manager works in collaboration with the Management Team under the guidance of the Owner/Franchisee. They are responsible for managing their time, planning and organisation of their work to meet Key Performance Indicators.
PRIMARY RESPONSIBILITIES
Conducting or arranging the conduct of holistic client and environmental assessments the contracting or development of Care Plans that maximise client choice, independence, wellbeing and dignity.
Conduct initial interviews with prospective HCP clients and their representatives detailing the scope of services, client expectations and documenting relevant information meeting the Aged Care Quality Standards (2019).
Develop and maintain client budgets, ensuring monthly reports are distributed to each client.
Conduct regular reviews of Care Plans to ensure that optimal supports are maintained to meet changing needs.
When necessary, arrange for clinical reviews to be conducted by a Registered Nurse.
Liaise with scheduling staff regarding the selection of appropriate staff for client service provision, ensuring familiarisation and matching of individuals.
Ensure that clients are made aware of their rights respecting their privacy, confidentiality, dignity, fulfilling duty of care requirements.
Assess the ability of potential clients to pay their contributions regularly and maintain associated records.
Promote continuity of care for clients through attending case management meetings and effective liaison with medical practitioners, hospital, allied health and other agencies involved as appropriate.
Contribute to and participate in the Clinical Governance Reporting and meetings.
Ensure a high level of flexible service provision through the development, understanding and maintenance of the impact of complex needs in relation to consumer directed care.
Communicate effectively with clients, carers and family recognising and respecting individual differences, backgrounds and beliefs.
Maintain accurate and timely client records and statistical data.
Contribute to the effective functioning of the business by following agreed procedures, policies and guidelines for service provision and in the identification of gaps within the program.
Ensure effective functioning of Senior Helpers reporting requirements (including DHS).

SELECTION CRITERIA
Degree in nursing or 2 years in a Case Management role with the Home Care Package program.
Understanding of the needs of frail aged persons in the community.
Broad understanding of the health and aged care system.
Understanding of financial and statistical reporting requirements.
Computer skills and knowledge of office software packages (MS Office) and the ability to learn propriety databases (Visual Care).
Familiarity with internet use and email.
Understanding of the Aged care Quality and Safety Standards.
Current registration with AHPRA.
Current National Police Check (within 3 years).
Unrestricted drivers’ license.
Current Working with Vulnerable Persons’ Card.
Desirable
Degree in Nursing.
Completion of post-secondary Case Management studies/qualification.
Other Requirements
Pass recurring 30-day probation review with Director/s.
Job Types: Full-time, Part-time, Casual
Pay: $70,000.00 – $90,000.00 per year
Expected hours: 38 per week
