Key Responsibilities:
Providing care for clients in their homes, promoting independence and dignity
Developing and implementing individualised care plans to meet client needs
Collaborating with healthcare professionals and other stakeholders to ensure seamless care delivery
Maintaining accurate records and reporting as required
Requirements:
Current registration with AHPRA
Satisfactory Australian National Police check
Working With Children's Check
Current first aid and CPR certificate
Driver's licence and reliable presentable car
