Grata Care Services, a National Home Care Package provider, is seeking a dedicated and consumer focused Care Manager to join it's Victorian team. This role will see you playing a pivotal role in managing a consumer's Home Care Package by coordinating and facilitating the delivery of high-quality consumer directed care. Work closely with our support structures to ensure a high quality care, which is responsive to consumer needs, compassionate and aligned with our organisational goals.
As part of this role, you will manage an existing portfolio of Home Care Package clients in the North and North West of Melbourne whilst also working alongside our business development team to facilitate on boarding of new Home Care Package clients.
This is a Hybrid role with work from home options but also a requirement to travel to client's home and into head office.
Key Responsibilities
Case Management of existing Home Care Package consumers in accordance with Aged Care quality standards and funding requirements
Coordination or high quality care consumer directed care
Internal and external stakeholder management
Responding to service inquiries and growth leads to support growth of Home Care Package portfolio.
About you
Demonstrated experience with management of Home Care Packages
Strong administrative skills with excellent time management
Ability to build and foster relationships with internal and external stakeholders to enable ongoing management and growth of Home Care Package portfolio
Working rights within Australia
Current NDIS Clearance and Police Clearance
Valid drivers license
