Job Description:
We are seeking a skilled Patient Care Coordinator to manage the referral process, ensure timely and efficient delivery of care, and develop strong relationships with outside sources. Your expertise in navigating complex Home Health cases, including IVs, TPNs, and Enteral Feedings, will be invaluable in providing high-quality patient care. You will also be responsible for:
Verifying client information and assigning cases.
Coordinating complex Home Health cases.
Triage and providing verbal and telephonic medical advice to current Home Health patients.
