Job Description
POSITION SUMMARY
The Hospice Licensed Practical Nurse (LPN), under the direction of the RN, supports the development, implementation and evaluation of the plan of care in collaboration with the disciplinary team to create the most positive Hospice experience for patients and families.
PRIMARY ACCOUNTABILITIES
• Provides nursing care and prescribed treatments for patients in their home, according to the physician's plan of care as supervised by Registered Nurse (RN).
• Ensures appropriate utilization of visits and services for care under the direction of the RN as noted through proper documentation completed at time of the visit.
• Reports alterations to the plan of care as indicated by changes in observed condition of patient to the appropriate RN.
• Assures the safety of the treatment procedures, patient, and environment at all times, while actively participating in Quality Improvement processes.
• Complies with State and Federal Regulations, The Joint Commission (TJC) standards and adheres to all Health First and Hospice of Health First Policies and Procedures in reference to delivery of Hospice Services.
• Assists in organization of patient care, scheduling and documentation reviews as requested by supervisor.
• Notifies leadership of any change in their projected scheduled visits, communicates any adverse findings to patient condition and documents findings in the medical record to leadership and Case Management.
• Meets or exceeds Patient Satisfaction goals as established every year, utilizing supplies in a cost-effective manner, and adhering to productivity requirements
