Job Description
Overview:
As a Licensed Practical Nurse (LPN), you will play a vital role in the overall operation and optimal quality of care for our residents on assigned units. You will be responsible for providing resident assistance with daily living activities and administering medications while overseeing staff.
Responsibilities:
Holds daily shift report with nursing assistants and reviews written daily assignments for each resident.
Makes rounds on assigned residents at least once every two hours.
Administers medication and treatments according to physician's orders, and assumes responsibility for ordering medications.
Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and updates these plans as needed.
Assists attending physicians with rounds and special procedures as required.
Transcribes, documents, and implements physician's orders.
Contacting resident's physician and family regarding changes in condition or accidents/incidents, and notifies supervisor covering.
Completes yearly evaluations of nursing assistants, documenting strengths, weaknesses, and areas for improvement.
Documents disciplinary concerns for nursing assistants, including reprimands, warnings, and recommendations for suspension or discharge.
Participates in hiring, interviewing, and orientation of nursing assistants.
Aids in Quality Assurance and Performance Improvement (QAPI) process.
Qualifications:
Practical Nurse licensed in the state of employment.
Minimum one year of education or experience in geriatric nursing or rehabilitation preferred.
Knowledge of nursing theory and practice, including administration of medications.
Ability to read, write, and comprehend English.
CPR certification.
Work Environment:
This job operates in a healthcare setting, requiring regular walking to various locations.
May be exposed to viruses, diseases, and infections from patients and specimens.
May require extended periods at a video display terminal.
May experience traumatic situations, including psychiatric and deceased patients.
Physical Requirements:

While performing duties, the employee is regularly required to talk and hear.
Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
This position is very active, requiring standing, walking, bending, kneeling, stooping, crouching, crawling, and climbing all day.
The employee must frequently lift or move objects and patients weighing over 50 pounds.
