Location: Seattle, WA (98144) – On-site
Compensation: $62,920 - $92,121 per year
The Opportunity:
We are seeking a Licensed Practical Nurse (LPN) for a unique role that blends clinical excellence with care coordination. In this position, you won't just be "working a floor"—you will be managing the continuum of care for high-risk geriatric patients. This is a permanent, full-time position in a collaborative outpatient setting.
Key Responsibilities:
• Care Transitions: Be the primary point of contact for patients transitioning from hospital to home.
• Clinical Triage: Perform patient assessments, manage telephone inquiries, and coordinate care across various services.
• Direct Nursing: Administer immunizations, perform lab work, and assist with clinical procedures.
• Education: Empower patients and their families by providing clear instruction on medications and ongoing treatment plans.
Qualifications:
• Active WA State LPN License.
• 1+ year of clinical nursing experience.
• Experience with Geriatrics, PACE, or Outpatient clinics is highly preferred.
• Proficiency in Epic or similar EHR systems.
• Strong critical thinking skills and the ability to prioritize in a fast-paced environment.
Schedule:
• Stable Monday–Friday schedule (Core hours 7:30 AM – 6:30 PM).
