Overview

Duties
Respond promptly to on-call nursing needs, providing assessments and interventions for patients in their homes or care facilities.
Conduct comprehensive patient evaluations, including vital signs, physical examinations, and medication administration.
Manage complex cases involving IV infusion, catheterization, wound care, and pain management, ensuring adherence to clinical protocols.
Collaborate with physicians, case managers, and family members to develop and update individualized care plans.
Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or Cerner, maintaining compliance with HIPAA regulations.
Provide education and support to patients and caregivers on medication management, disease processes like dementia or Alzheimer's care, and safety precautions.
Assist with triage by assessing urgent needs and coordinating appropriate emergency or outpatient services when necessary.
Experience
Valid Licensed Practical Nurse (LPN) license with current state licensure.
Prior experience in home health nursing or medical-surgical units demonstrating strong patient assessment skills.
Critical care experience preferred, including ICU or emergency room backgrounds that involve acute pain management and airway management.
Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation.
Knowledge of ICD-10/ICD-9 coding for billing and documentation purposes.
Experience working with individuals with disabilities, dementia, or developmental disorders enhances patient-centered care delivery.
Certifications in Basic Life Support (BLS) and sterile technique are required; additional certifications such as IV infusion or phlebotomy are advantageous.
