Overview
Provides program support in coordination of the Helen Kaplan Program ICF. Ensures staff training on the individual’s health and well-being, and creates, updates and trains on agency policy. Works in compliance with OPWDD regulation for individuals residing in intermediate care facilities while providing for optimal health of individuals.
Responsibilities
Responsible for assisting with medication pourings for the HKP ICF AMAP staff; including completion and assessment of/ competency checklists for vital signs; Diastat, Epipen, tube feeding, and insulin administration, etc. Collaborates with the HKP Assistant Director of Nursing in creating, obtaining, and completing a weekly schedule of AMAP pourings pending for the program.
Ensures CPR and OPWDD new nurse training class for each nurse is completed and updated as required. Assist in training new nurses.
Trains and retrains AMAP staff regarding the health of the individuals they serve, including writing and training on guidelines and updating plans of nursing to ensure staff understanding and compliance. Collaborates with the Assistant Director of Nursing in creating, obtaining, and completing a weekly schedule of new trainings and re-trainings pending for the program.
Functions as a float nurse during nursing vacancies/time off; including overseeing a caseload when necessary.
Assists in creating new nursing policies and procedures for the HKP ICF when needed and revises current policies and procedures, responsible for training on policies. Participates in the Policy Review Committee.
Confers with the HKP Assistant Director Nursing Citizens through meetings to exchange information, receive instructions, and offer updates.
Spot checks MARs and nursing documentation.
Confers with the HKP ICF team to discuss present individual’s medical status as needed and to ensure ongoing communication and training of pertinent information.
Attends Nursing staff meetings and management meetings, as needed, to communicate medical information and seek recommendations to improve AMAP oversight and medical care to HKP ICF individuals.
Confers with medical providers (pharmacies, psychologists, lab supply vendors, and physicians) by telephone and via scheduled meetings, to ensure the delivery of quality and cost-effective health care services.
Supports and participates in on-call system. Creates on-call schedule and guidelines.
Participates in meetings regarding screening new potential admissions.
Oversees and assists in coordinating hospital discharges
Maintains statistics on all individual hospitalizations as a means of utilization review.
Orders all supplies for the program including tube feeding supplies and over the counter medications.
Assists HKP Assistant Director of Nursing with Nursing schedule.
Completes weekly rounds with Medical Director in the absence of the RN
Adheres to all regulations, agency policy, and procedures, the employee handbook, and agency and program training requirements.
Reports to Justice Center as a mandated reporter if there is reasonable cause to suspect abuse, neglect, or a potentially hazardous work environment. Additionally, follows all agency incident policy and procedures.
Ensures that work-related safety procedures are in place and are being adhered to by staff.

Ensures that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.
Arrives to work by start of scheduled shift, adheres to agency policy and procedures in regard to call offs and overtime to ensure quality support is provided to people each day.
Performs additional related tasks and special projects as requested by your supervisor.
Qualifications
NYS Registered Nurse license; 3 years’ experience in clinical nursing working with the developmentally disabled; supervisory experience preferred. Current CPR training/ certification required.

Description:
The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients. Duties include but are not limited to:
• Identifies patients with barriers to a safe discharge plan.
• Completes psychosocial assessments on all patients.
• Documents all clinical assessments, notes, and referrals in Epic.
• Completes screens and coordinates all new skilled nursing facility and rehab facility referrals.
• Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and ****** assault.
• Educates patient/family about post-acute care options and the role of choice.
• Educates patient/family about managed care and payor issues
This is a full-time day shift position 9:00am-5:00pm Monday-Friday including every 4th weekend, both Saturday and Sunday, located at NewYork-Presbyterian Brooklyn Methodist Hospital.
Preferred Criteria
• At least one year of experience in an acute care setting
