About the Opportunity
The City of New York's Department of Health and Mental Hygiene is seeking a skilled social worker to join its team as a Case Monitor for the Bureau of Mental Health's Assisted Outpatient Treatment (AOT) program. As a key member of our team, you will play a critical role in ensuring the successful implementation of treatment plans for individuals with serious mental illness.
Job Summary
We are looking for a highly motivated and organized individual to serve as a Case Monitor for our AOT program. The ideal candidate will have a strong background in social work, excellent communication skills, and the ability to work effectively in a fast-paced environment. Key Responsibilities
Monitor consumer adherence to court order and treatment plan.
Verify consumer services monthly by completing the monthly service verification.
Ensure that treatment plans are complete timely.
Document any changes to the treatment plan as needed and in consultation with AOT psychiatrists during the order.
Participate in case conferences to discuss consumer eligibility for AOT as well as progress under the court order.
