Monitor mental and physical health and respond proactively to any changes in condition or risk. Work collaboratively with community teams to ensure seamless transitions between inpatient and outpatient care.

Offer education and support to families, colleagues and carers, ensuring they feel informed and involved in the care process. Participate in clinical audits, continuing professional development, and quality improvement initiatives to enhance patient outcomes.
